Overview: Neutrophils are a type of white blood cell that helps fight infection. In certain autoimmune disorders, the immune system creates “Anti-Neutrophil Cytoplasmic Antibodies” (ANCA) that mistakenly target these helpful cells. This attack causes the neutrophils to release toxic substances that damage the walls of small blood vessels. The test usually looks for two main patterns/types:

  1. c-ANCA (Cytoplasmic): Primarily targets an enzyme called PR3.

  2. p-ANCA (Perinuclear): Primarily targets an enzyme called MPO.

Clinical Significance:

  • Granulomatosis with Polyangiitis (GPA): Formerly known as Wegener’s Granulomatosis. Highly associated with a positive c-ANCA (PR3) result. Affects the sinuses, lungs, and kidneys.

  • Microscopic Polyangiitis (MPA): Often associated with a positive p-ANCA (MPO) result. Primarily affects the kidneys and lungs.

  • Ulcerative Colitis: Atypical p-ANCA results are sometimes seen in patients with Inflammatory Bowel Disease (IBD).

  • Churg-Strauss Syndrome: Now called Eosinophilic Granulomatosis with Polyangiitis (EGPA), also linked to ANCA.

When is this test recommended?

  • Kidney Issues: For patients with unexplained kidney failure or blood in the urine (glomerulonephritis).

  • Lung/Sinus Symptoms: Chronic sinusitis, coughing up blood, or lung nodules that don’t go away with antibiotics.

  • Systemic Symptoms: Unexplained fever, weight loss, and muscle aches combined with organ damage.
