Overview: When the body is infected with Brucella, it produces two main types of antibodies: IgM (early stage) and IgG (late stage/long-term). This “Total” test measures both simultaneously. It is often used as a sensitive first step; if this test is negative, it is highly unlikely the patient has Brucellosis. If positive, further testing (separating IgG and IgM) is done to determine if the infection is new or old.

Clinical Significance:

  • Maximum Sensitivity: By detecting both antibody types, this test rarely misses an infection, making it the ideal initial screen.

  • Diagnosis of Brucellosis: It helps confirm the diagnosis in patients with symptoms like recurring fever, night sweats, and joint pain, especially those with a history of animal contact or raw dairy consumption.

  • Exclusion: A negative result is very reliable for ruling out the disease in patients with “Fever of Unknown Origin.”

When is this test recommended?

  • As a primary screen for patients suspected of having Malta Fever.

  • For patients with vague, long-term symptoms like fatigue, depression, and body aches who live in areas where Brucellosis is common.

  • As a follow-up to a positive rapid slide test to confirm the result with a more accurate titre.
